

Field Trip Requests

Step by Step Guide:

Step 1: Field Study Form A. Once you have a general idea of what you want to do and a proposed date (check school calendar first), submit Field Study Form A. Wait for an email approval of your Field Study before taking further action.

Step 2: Request transportation or bus.

Step 3: Submit Field Trip Form B.

Step 4: Add Event to School Calendar.

Step 5: Send home a letter with permission form.

Step 6: Prep items for bus driver
1. List of students going on trip. Last Name, First Name, DOB.
2. List of chaperones (some bus drivers don’t really care about this, as they have the chaperone agreements.​​
Helpful Guide for Your Planning

Email parents and students. Remind them about important items, such as:

What students are to wear.

When to get to the school.
-
Remind parents to check in through LOBBYGUARD at the office.

What time the students will come back.
-
Lunch or lunch money needed.
-
Anything else students should bring or do.
-

Prep a folder for yourself.

List of kids going on trip. Possibly include a column with check boxes for permission form in and money paid. Add a few columns for checking them onto the bus, as you will do this repeatedly.

Make sure all permission forms have emergency contact info on them and are signed properly.

Check them against your list.

Hunt down students who haven’t turned in permission slips.

Day of the Trip

Get the trip bag from the nurse. It will include a general first aid kit and any medications for students going on the trip.

Permission Slip and Attendance Folder (Keep with you throughout the trip.):

Make sure your list of students is accurate and matches permission slips in hand.

If a child has no permission slip, cross the name off the list and notify attendance that he/she is not actually going on the trip.

Bus entry

You enter first and check in with the bus driver. Give him/her all the documentation you prepared – be sure the list is accurate and that you’ve removed any kids not going after all.

Check off each student on your own list as they enter the bus

YOU MUST CHECK OFF EACH STUDENT EVERY TIME THE GROUP RE-ENTERS THE BUS. STUDENT SAFETY IS #1 PRIORITY, ESPECIALLY THAT THEY ARE ACTUALLY WITH YOU!
